Primary Care Network

You may have heard through health news that all GP practices are being encouraged to become members of primary care networks (PCNs), with the aim of improving overall patient care by closer collaborative working with other local practices in their locality.

 

In this regard the practice will become part of such a network in the near future forming a primary care network with Haxby Group practice and Front street Surgery.

 

It is important to recognise within this that The Old School Medical Practice continues to function as an autonomous independent practice.

 

In order to optimise health care it may be necessary to share patient health data between health providers within these networks but this data will only be shared as necessary to benefit an individuals health care needs or the health care needs of the population. Examples include out of ours and extended hours working and access /specialist clinics /digital health services and Minor Injury Services.
